  3. PURPOSE: To motivate participants to adopt and sustain a healthier lifestyle to manage and/or prevent chronic disease OBJECTIVES: 1. To build on participant knowledge of previous workshops and finding balance 2. To review the impact of chronic diseases and their similar relationships to physical activity 3. To identify risk factors and motivate participants to modify these to manage/prevent chronic disease 4. To explore progress with lifestyle goals and plans for sustainability

Creating a Safe Space As we will be discussing your experiences and feelings as you reflect on the workshop series, we ask that you draw on principles of safety and endeavour to make this is a safe place for all participants to share and be heard. Confidentiality, Listen with respect, Self-care

How are you getting along with your lifestyle goals? 3. What disappointments have you faced? 4. What have you put in place to support achieving the goal? Copy questions before going to breakout rooms Decide on a reporter The reporter will report back on 1 highlight from the group

  10. Chronic Disease Discussion 20 minutes Three groups Cancer, Cardiovascular Disease, Diabetes Brainstorm for 10 minutes and have reporter record on word doc: What are the risk factors? What is the impact on lifestyle, signs and symptoms? What are the management and prevention techniques (to maintain wellness now and in the future) Instructions: 1. 2. 3. Copy questions before going to breakout rooms Decide on a reporter The reporter will report back by sharing the word doc. Compare notes in large group any similarities? differences? 10 minutes

  13. Physical Activity Upper Back Flexibility Sit in an armless chair feet flat on the floor shoulder width apart 1. Hold arms in front of you at shoulder height with palms facing outward 2. Relax your shoulders keeping upper body still, and reach forward with your hands. Stop when you feel a stretch or a slight discomfort. 3. Hold position for 10-30 seconds 4. Sit back up 5. Repeat 3-5 times 6.

I am important Be mindful and accountable reinforced by the program We are all in the same boat and will support each other on our journey I am responsible for my destiny and the session has helped me to regroup and helped reinforce that I am responsible, providing tools to stay on track Its never too late to start or restart Recording my progress is helpful I need to see a change happening and its helpful. I got some stars!! I am not alone I need to take responsibility we will support each other Given me the feeling that I can do this an empowering feeling.
