  3. Stroke Facts Stroke Facts 1 in 6 will experience a stroke in their lifetime (for women: 1 in 5) Stroke is the #1 cause of long-term disability 90% of the time, a bystander calls 911 Up to 80% of strokes are preventable

  4. What are the signs?

  5. Is there a sudden loss of vision in one or both eyes? Is there a sudden loss of balance?

  6. Predicament Ischemic strokes (87% of all strokes) can be effectively treated with a targeted medication Must be administered within 4.5 hours of onset 75% miss the treatment window Calling 911 is critical! 2 million brain cells die every minute!

Studies suggest 1/3 of patients with ischemic strokes first called their doctor for an appointment* 2. 80% of population see a medical professional annually; more often if high risk 3. Patients trust their doctors 4.   10. Stroke Smart Medical Practice Elements* 1. Train all office staff to spot strokes and follow the practice protocol if a stroke is suspected (suggested: annually) 2. Provide Stroke Smart education and materials for all patients (i.e., wallet cards, magnets, posters, videos) 3. Identify high risk patients and provide (intentional) education and materials 4. Incorporate Stroke Smart script in voicemail to spot and instruct calling 9-1-1 (suggested element) 5. Track metrics on Stroke Smart effectiveness *Approved by Virginia Stroke System Task Force: Jan 2023

Health System Activities: Piloting implementation of all elements in a primary care site Developed online Stroke Smart course to train staff Stroke Smart Information Sheet provided to all patients Coding Epic system to flag high risk stroke patients Phone system voice recording has been completed Independent Practices: Patient education materials sent to 16 independent practices and wellness centers through our marketing efforts

  12. Stroke Smart Medical Collaboration: Facilitate the adoption of SSMP elements in practice, minimizing duplication of efforts and sharing best practices Identify opportunities to expand the initiative to practices and systems with an emphasis on areas with high stroke prevalence
