  5. Challenges Time/Scheduling Ours and theirs Limits with what Transitional Care Units (TCUs) can do at discharge Bundled/risk-based payments haven t become common yet

  6. Opportunities Overall positive reception Started the relationship building Lyngblomsten

Lyngblomsten Partnership Project Applying for a Live Well At Home grant from State of Minnesota in partnership with Lyngblomsten If grant is successful, Lyngblomsten/Meals on Wheels will provide 10 meals following TCU discharge hot daily or frozen weekly paid for by grant Lyngblomsten nurse would enroll the patient in MOW MOW program will follow up before the 10 days is over to see if client wants to continue meals or not (private pay) May offer an enhanced well-being check during the 10 days of meals

  8. Key Learnings Grant-funded pilot projects seem to be the best bet to start this work. We needed to start small to focus on a specific population. Partnerships will be key to accessing grants and other new initiatives.

  9. Next Steps Cross our fingers for Live Well At Home (LWAH) grant Continue TCU outreach Continue networking Begin TCU outreach in Hennepin County Continue collaboration with Metro Meals on Wheels for metro-wide health care integration
