  1. Olecranon Apophyseal Ossification System Guide for Radiographic Evaluation

  2. Overview Stage 0 no apophyseal ossification Stage 1 one small, circular/oval ossification Stage 2 one larger, oblong ossification Stage 3 two ossifications that do not overlap Stage 4 two ossifications that overlap Stage 5 one fused ossification that is rectangular proximally Stage 6 one fused ossification that is pointed/rounded proximally Stage 6.5 one fused ossification that is partially fused to the volar ulna metaphysis Stage 7 completely fused ossification

  3. Stage 0 No ossification of olecranon apophysis

  4. Stage 1 Stage 1 One, small circle/oval ossification vs Stage 2 Oblong/longer ossification Stage 2

  5. Stage 2 Stage 2 Relatively large ossification Variable shape (noncircular) vs Stage 5 rectangular top at level of olecranon articular surface Stage 5

  6. Stage 3 Stage 3 Two ossifications that do not overlap/touch at any point vs Stage 4 Two ossifications that overlap/touch Stage 4

  7. Stage 4 Stage 4 Two ossifications that overlap/touch at any point vs Stage 6 smooth single apophyseal piece Stage 6

  8. Stage 5 Stage 5 Rectangular top at level of olecranon articular surface vs Stage 6 Rounded/peaked tip posterior-proximally Stage 6

  9. Stage 6 Stage 6 Rounded/peaked apophyseal tip No evidence of fusion with metaphysis Able to visualize entire apophyseal line vs Stage 6.5 partial fusion with metaphysis Stage 6.5

  10. Stage 6.5 Stage 6.5 Partial fusion Small radiolucency between epiphysis and metaphysis posteriorly vs Stage 7 Complete fusion Stage 7

  11. Stage 7 Smooth posterior ulna Complete fusion Physeal scar may be present
