PKB (Patients Know Best) is a patient-held record platform that empowers patients to access and manage their health information effectively. It enables patients to track symptoms, connect devices, share data with clinical teams, and engage in self-care. With features like communication tools, symptom trackers, test result sharing, device connectivity, and integration with the NHS app, PKB improves patient outcomes, reduces unnecessary appointments, and promotes remote monitoring for better healthcare management.

What is PKB? PKB is a patient held record that enables patients to access information held about them across the health and care economy. Patients can add to this record by tracking symptoms, connecting devices and recording outcomes. PKB can support patients to manage long term conditions whilst at home through access to a wealth of educational materials, interactive care plans and support. The platform can provide the patient reassurance, support and confidence in their health care by improving knowledge and communication. Other benefits include a reduction in phone calls and appointments and digital letters to mention just a few. A contract is in place with PKB which means the platform is now available at no additional cost to Humber and North Yorkshire Health and Care Partnership.
