Evergreen Valley College NCLEX Application Review Updated: 2/8/19
Prepare the online NCLEX application from https://www.breeze.ca.gov/The application may be submitted two weeks prior to graduation date (December 4). A Checklist is included in the packet to assist applicants in completing the application The California Department of Justice will no longer accept fingerprint cards from applicants residing in California
Review of the Application http://www.rn.ca.gov/pdfs/applicants/exa m-app.pdf Fee is $200.00
Breeze Application https://www.breeze.ca.gov/ Create a User Account, if you have not already done so. Once you have created a User Account, you can begin entering application information by selecting: 1 RN Initial Exam Application from the dropdown box below Start a New Application or Take an Exam. Please read the questions carefully before responding. Please ensure all application documents, including application, fingerprints, and Pearson VUE are listed as it appears on your Driver s License. At the end of the application process, you will have the opportunity to attach documents to the application. Please attach as much information as possible to reduce paperwork, speed the review process, and reduce the need for deficiency letters. You can scan and attach the following documents to your online application: Completed Examination Identification Form with Photo attached Completed LiveScan Form (if fingerprinted in California) Completed Request for Accommodation of Disabilities form, if applicable For students disclosing previous conviction(s) and/or discipline(s), you can attach: Letter of explanation (signed and dated) Letter(s) of recommendation/support Any other rehabilitation information listed in section II of the Exam Application Instructions You must still have the following documents submitted via US Mail: Completed Request for Transcript form with certified Transcripts sent directly from the nursing school Completed Individual Candidate Roster sent directly from the nursing school Completed Fingerprint Hard Card(s), if applicable Certified court and arrest documents, if applicable Rehabilitation documentation, if applicable
Reporting Prior Convictions and Discipline Includes traffic violations of $1000 or more Applications are handled on case by case basis If you are unsure, safer to disclose Considerations made by Board: Age of incident Singular vs. multiple incidents Pattern of behavior Age at time of incident Elements of the incident, i.e, sexual in nature, abuse, children involved, chemical abuse, mental illness, etc. Please keep in mind that if you have a background issue, your NCLEX application can take up to 6 months for processing.
Do not register with the testing service, Pearson VUE until after you have received an eligibility notice from the Board. If for some reason you are found not eligible for the NCLEX-RN examination, you could lose the $200.00 registration fee. When the registration is matched with the Board s eligibility, an Authorization to Test (ATT) is sent from Pearson VUE. All personal information must match exactly with the application submitted to the Board (e.g., name, Mother s maiden name, address of record, e- mail address, date of birth, U.S. Social Security Number), or an ATT will not be issued.
Upon receipt of the ATT you can schedule a testing appointment at a Pearson Professional Center, of your choice, by telephone or website. Remember that their identification (e.g., driver s license, passport, etc) provided at the test site must match the name provided to the Board, testing vendor and the ATT.
The Board no longer accepts applications unless it contains a valid U.S. Social Security Number. Don t forget to send in your 2x2 passport photo (its on the checklist) You must have completed all of your GE Requirements to submit the forms to my office and to apply to the Board. If you have not completed all of your classes, please send me an email with what classes you have to finish and your plan for completion.
Program Roster with Director approval of graduates will be sent to the Board December 4th Evergreen will upload your transcripts to the BRN/Cloud approximately 2 to 3 weeks after graduation Board is indicating that you should receive your NCLEX test date in January If you have not yet completed your coursework for graduation, please let me know, you can not apply for NCLEX until completed
The Chancellors Grant has afforded us to pay for all of you to do the Kaplan review. Kaplan Review All of you will receive an email from Kaplan with the choice of review classes you can take.
Get your reference letters now from your instructors for employment reference letters. You can request via email to your instructors. Keep a copy of your preceptor evaluation as a lot of employers are requesting this information Often students are asked about our program hours from prospective employers, you can access them on our website
