Objectives Objectives By the end of session, the participants will be able to describe the process of MPDSR in the hospitals, identify the members of MPDSR Committee in the Hospital, District, Regional and National levels and identify the roles and responsibilities of individuals and MPDSR Committee in the Hospital level MPDSR process.

Responsibilities of Hospital MPDSR Committee Responsibilities of Hospital MPDSR Committee A. To review maternal and perinatal deaths in hospital. B. To ensure that all maternal deaths have been notified and properly reviewed. C. To ensure proper management of maternal and perinatal mortality data. D. To give necessary support to the hospital team by summarizing the results obtained. E. As per the review, make necessary coordination with the state, health office and local level MPDSR committee. F. To make action plans and mobilize resources to implement the suggestions received. G. Continuous follow up to ensure implementation of action plan. H. To present the facts and suggestions obtained from review to the health office, Province and Centre.

To cooperate and coordinate with various stakeholders in order to enhance the quality of service in the hospital. H. In the annual MPDSR reviews: To present the facts. To review the MPDSR program in the annual and other review programs. G. Make action plan after review, implement and follow up regularly. H. To inform the concerned body about the death of the mother. I. MPDSR To review whether the recommendations of the previous meeting have been implemented in each meeting of the committee. J. Provide computer and laptop, enter information online and make response plan and give responsibility to improve the quality.
Role of Attending Doctor and/or Nurse Role of Attending Doctor and/or Nurse Identify maternal and perinatal deaths within the hospital Notify death to the Doctor In-charge / committee Complete the MDR form within 24 hours and PDR form within 72 hours of death in coordination with the attending staff Support the hospital based MPDSR-Committee to review maternal deaths within 72 hours and perinatal deaths every month

Role of District MPDSR Committee (At HO) Role of District MPDSR Committee (At HO) Facilitate to conduct MPDSR capacity building activities Evaluate & verify online data from local level & hospitals Facilitate planning & implementation of the program Assist hospitals to review deaths, receive & implement suggestions Implement issues that cannot be resolved at local level Monitor notification, identification & VA at local level Facilitate utilization of budget allocated at hospital level
Role of Provincial MPDSR Committee Role of Provincial MPDSR Committee Decide the activities & budget for expansion & continuous operation Provide resources to improve quality Ensure review processes are implemented Provide training to HO, Local levels, hospitals as per need Regular monitoring, evaluation & review of online data Forward suggestions from local level & HO to centre for necessary assistance including policy reforms Prepare annual action plan & report based on MPDSR data
