Webinar Topics Today we are pleased to discuss the Making Advances in Mammography and Medical Options for Veterans Act (MAMMO Act) We will discuss the approach from the Department of Veterans Affairs (VA) to implement the MAMMO Act. We will focus on two sections of the law: Section 101: Strategic plan for breast imaging services for Veterans Section 105: Mammography accessibility for paralyzed and physically disabled Veterans

Scavella, MD, FACP, FACHE Assistant Under Secretary for Health for Clinical Services Veterans Health Administration Dr. Scavella is the Assistant Under Secretary for Health for Clinical Services and Chief Medical Officer at the Veterans Health Administration. She has served in this position since January 2, 2022. Dr. Scavella leads the Making Advances in Mammography and Medical Options for Veterans Act (MAMMO Act) implementation efforts in VA.
Background On June 7, 2022, the Making Advances in Mammography and Medical Options for Veterans Act (MAMMO Act) was signed into law. The law has multiple sections with requirements for VA and is organized into two topic areas: Breast cancer screening and early detection Breast cancer partnerships for research and access to care
VAs Commitment to Implementing the MAMMO Act The MAMMO Act builds upon the Department of Veterans Affairs (VA s) commitment to ensuring all Veterans have access to high- quality breast imaging services, either through in-house Veterans Health Administration (VHA) mammography programs or community care. VA s implementation of the MAMMO Act is a collaborative effort with Federal and community partners. Who does the MAMMO Act affect? All Veterans, both women and men Veterans living in rural areas Veterans with paralysis, spinal cord injury or disorder, or physical disabilities Members of the Armed Forces and transitioning service members with breast cancer
Key Elements of the MAMMO Act Three-year telescreening mammography pilot program Strategic plan for breast imaging services Study on availability of and an expansion of testing for breast cancer gene among Veterans Mammography technology at VA facilities Accessibility to mammography and breast imaging services in VA and VA community care for paralyzed and physically disabled Veterans Partnerships with the National Cancer Institute and collaborations with Department of Defense on breast cancer treatment and research
Section 101: Strategic Plan for Breast Imaging Services for Veterans VA s Strategic Plan for the Provision of Breast Imaging Services will outline the plan for the Veterans Health Administration (VHA) to address the evolving needs of all Veterans, both female and male. The strategic plan will address: Geographical disparities of VA imaging and the use of breast imaging in the community Use of digital breast tomosynthesis Needs of male Veterans who require breast cancer screening services. VA is aligning to strategic plan around the four primary goals of Quality, Access, Outreach, and Veteran Experience. The strategic plan is due to Congress June 7, 2023. Section 101: Strategic Plan for Breast Imaging Services for Veterans The strategic plan will provide recommendations related to equity and access for breast imaging. VHA Mammography Programs By Fiscal Year (FY) 80 69 69 70 6466 70 VA is analyzing its current state and is identifying opportunities to better meet the needs of Veterans for breast imaging services, both in-house and in the community. 60 61 Number of Programs 5254 60 48 4345 50 38 37 40 30 VA currently offers mammograms on-site at 70 VA facilities, and for those women receiving care at sites without mammography, we offer screening at community facilities. 20 10 0
Section 105: Mammography Accessibility for Paralyzed and Physically Disabled Veterans The efforts in response to MAMMO Act Sec. 105 will ensure providers of breast imaging services are equipped with the knowledge, equipment, infrastructure, training, and support to provide state of the art, accessible and equitable care for Veterans with a spinal cord injury or disorder and those living with paralysis or other physical disability. Key requirements: Study on the accessibility of breast imaging services at VA facilities for Veterans with a spinal cord injury or disorder and those living with paralysis or other physical disability. Update of policies and directives to improve accessibility and care for Veterans with a spinal cord injury or disorder in the community. 2022 2023 2024 Planning and Design Implementation and Policy Revisions Study Findings and Report to Congress Expected MAMMO Act Section 105 Implementation Timeline
Section 105: Mammography Accessibility for Paralyzed and Physically Disabled Veterans The VA Spinal Cord Injuries and Disorders (SCI/D) System of Care (SoC) offers inclusive, coordinated, lifelong care provided by interdisciplinary teams of SCI/D experts in 25 regional SCI/D Centers ( Hubs ) and designated local SCI/D primary care teams at more than 120 other VA medical facilities ( Spokes ). Additionally, VA provides institutional long term care (LTC) to Veterans with six dedicated SCI/D LTC Centers across the system, which is not available elsewhere in the United States.
Section 105: Mammography Accessibility for Paralyzed and Physically Disabled Veterans VA Mammography Programs On-site mammography is offered at 52 Spinal Cord Injury and Disorder VA sites of care. The VA Imaging Design Guide provides guidance on accessibility design in mammography suites. VA Mammography programs obtained enhanced positioning aids with Office of Women s Health funding. Future plans include disseminating best practices and providing education on the needs of SCI/D patients.
Section 105: Mammography Accessibility for Paralyzed and Physically Disabled Veterans The MAMMO Act requires VA to update policies and directives to improve accessibility and care for Veterans with a spinal cord injury or disorder in the community. It also requires VA to: Confirm with the provider the accessibility of the breast imaging site; and Provide information to provider on best practices for screening/ treating Veterans with a spinal cord injury or disorder. An environmental scan is underway to identify the most feasible course of action to ensure providers are equipped to perform as the law intends in referring Veterans to community providers.
Breast Cancer Awareness Month About 1 in 8 women (about 13%) and about 1 in 833 men (>1%) will develop breast cancer over the course of their lifetime. In 2022, an estimated 287,850 new cases of invasive breast cancer are expected to be diagnosed in women in US.* Most common diagnosed cancer among women. VA offers mammograms for average risk women beginning at age 40.
