The Work It Out Lewisham resource can be found at: http://workitoutlewisham.co.uk/ Local Offer The Lewisham Local Offer is an online resource for families of children and young people with special educational needs and/or disabilities (SEND). The Local Offer aims to provide in one place - clear, relevant and up-to-date information, advice and support. This support covers education, health & social care for children and young people aged 0-25 within the borough of Lewisham with SEND. The Lewisham Local Offer can be found at: http://www.lewishamlocaloffer.org.uk/#/ Kooth Kooth provides confidential online counselling (delivered by British Association for Counselling & Psychotherapy accredited counsellors) for Lewisham young people living, attending school or with a GP in the Lewisham borough and who are aged 10 to 19 (up to 25 for children with disabilities). Access to online counsellors is available between the hours: Mon Fri from 12noon 10pm and Sat Sun from 6pm 10pm.   3. LEWISHAM SERVICES FOR CHILDREN & FAMILIES: AUGUST 2018 AREA SERVICE CONTACT The Health Visiting Service and Family Nurse Partnership are delivered by Lewisham and Greenwich NHS Trust. A single contact point is provided by the Health Visitor admin hub: Health Visiting & Family Nurse Partnership Health visitors are registered nurses or midwives with a specialist qualification in public health. They work with families with children between the ages of 0-5. There are five key stages in a child s development when you will be contacted by your health visitor: Before the baby is born (when you are around 30 weeks pregnant) New birth visit between days eight and 14 Six to eight week visit Seven to 11 month developmental review Two to two and a half years developmental review Each health visiting team runs a child health clinic, which you can attend for advice, support and weighing. You do not need to make an appointment to attend the child health clinics. Lewisham Mindful Mums groups are free for women to learn how to stay emotionally well, develop confidence and social connections during pregnancy and in the first year of birth. LEWISHAM SERVICES FOR CHILDREN & FAMILIES: AUGUST 2018 AREA CONTACT SERVICE Child and Adolescent Mental Health Services (CAMHS) If you have concerns about your child s mental health, it is advised that you speak with your child s GP or school, who will make a referral to the local CAMHS service, if appropriate. Community CAMHS CAMHS are a specialised multi-disciplinary service, who provide mental health interventions for more severe complex or persistent mental health disorders for children and young people up to age 18 (or up to 21 where a child is leaving care). If you are a professional that has concerns about a child s emotional / mental health and you are unclear about the referral process, you are advised to contact the CAMHS Duty Team at Kaleidoscope In Lewisham Community CAMHS are separated into six specialist teams: ARTs (forensic) LYPS (Emerging Psychosis) SYMBOL (Children looked after) Horizon (Generic referrals) Neuro-Development (Disabilities) Crisis Team (hospital admissions) Tel: 020 7138 1250. Health & Wellbeing For all queries relating to the Eating Disorder concerns, contact the Eating Disorder Service (based at South London and Maudsley NHS Foundation Trust) Tel: 020 3228 2545 where clinical staff are available between Monday-Friday, 9am-5pm(9am 6pm for referrals only) Eating Disorders Where there is concern in relation to an eating disorder it is advisable to complete medical investigations (blood, weight, height etc.) via the GP. LEWISHAM SERVICES FOR CHILDREN & FAMILIES: AUGUST 2018 CONTACT AREA SERVICE Young People s Health & Wellbeing Service The Young People s Health & Wellbeing Service is an integrated and accessible service for 10-19 year olds (up to 25 for young people with additional needs). The service is focused on reducing the occurrence and impact of the three main risks for ill-health in teenagers substance misuse, risky sexual behaviour and emotional wellbeing. LEWISHAM SERVICES FOR CHILDREN & FAMILIES: AUGUST 2018 CONTACT AREA SERVICE Place2Be London Head Office Place2Be 13 / 14 Angel Gate 326 City Road London EC1V 2PT Provides in-school mental health support and expert training to improve the emotional wellbeing of pupils, families, teachers and school staff to enhance the wellbeing and prospects of children and their families by providing access to therapeutic and emotional support in schools. Place2Be works in seven Lewisham Schools: Prendergast Vale, Deptford Green, Tidemill Academy, Brindishe Green, Brindishe Manor, Coopers Lane and Marvels Lane. Taking a 'whole school approach , school-based teams of professional counsellors and therapists build strong links with students, staff and wider school community to help develop a mentally healthy school environment. This is supported by specialist teams on safeguarding, special educational needs and disability and clinical supervision. They also provide health education, offer advice on smoking and diet, run clinics, give vaccinations and carry out simple surgical operations. A list of all GP surgeries in Lewisham can be found at: http://www.lewishamccg.nhs.uk/your-health/Pages/GP-surgeries-in-Lewisham-.aspx GP Extended Access (GPEA) Service GP Extended Access (GPEA) Service There are now more bookable GP appointments available in Lewisham, making it easier to get an appointment at a suitable time, including evenings, weekends, and bank holidays. The GP Extended Access Service is an extension of your usual GP practice. It is not a walk-in service you must make an appointment. 8am-8pm, Seven Days a Week The service is located on the ground floor of the main entrance building, in the yellow zone at University Hospital Lewisham, Lewisham High Street, London, SE13 6LH. Health & Wellbeing Community health services for children in Lewisham include: Community Paediatricians If you have concerns about your child s development, it is advised that you speak with your health visitor, your child s GP or their school (see pages 3 & 9 of this leaflet) who will make a referral to the local Community Paediatric Service, if appropriate. The community paediatric service addresses the developmental needs of children and young people and their families; especially those children and families in vulnerable groups e.g. children with disabilities. 10

LEWISHAM SERVICES FOR CHILDREN & FAMILIES: AUGUST 2018 AREA CONTACT SERVICE Community health services for children in Lewisham include: Referrals to the Children s Community Nursing Team can be made by anyone, including hospital staff, GP s, community staff and parents. All children and young people referred to the team must be registered with a GP in Lewisham, Lambeth or Southwark. Nursing The Children s Community Nursing Team provide caring and specialist nursing care to children in their own homes and in community settings (e.g. schools, nurseries & respite care facilities), across Lewisham, Lambeth and Southwark Tel: 020 3049 3780 Email LH.CCNT2@nhs.net Therapies The Children s Physiotherapy Service undertake assessments, treatments and various interventions for children and young people with neurological conditions and related diagnoses, genetic abnormalities and those with a neurological condition requiring post-operative management. Referrals to the Children s Physiotherapy Service can be made by all health professionals, including GP s, health visitors, and school nurses. Tel: 020 7138 1337 The Children s Occupational Therapy Team undertake initial assessments, reviews, group and individual intervention to help children achieve the skills needed for home and school independence. Parents/carers can make referrals to the Children s Speech & Language Therapy Team by contacting the department or by coming to a drop-in clinic. They can arrange for that person to be seen promptly at the clinic of their choice. Under 16s are fast tracked at the Waldron Clinic and Hawstead Road Clinic. Under 18s are fast tracked at the young persons clinics at Downham and Sydenham. Lewisham Contraception and Sexual Health Services There are 4 clinics across the borough including 2 young persons at Sydenham Green and has access to the Come Correct condom distribution service . LEWISHAM SERVICES FOR CHILDREN & FAMILIES: AUGUST 2018 AREA SERVICE CONTACT Special Educational Needs Service The majority of children and young people with SEND can have their needs met within their local education setting. However, a small number of children and young people may require an Education, Health and Care needs assessment (EHC needs assessment) in order for the local authority to decide whether it is necessary for it to make special educational provision in accordance with an Education, Health and Care plan (EHC plan). For more information about EHC plans, access Lewisham s Local Offer: http://www.lewishamlocaloffer.org.uk/#/ The team will undertake the EHC needs assessments process and put in place the EHC plan should it be assessed that this is required to meet your child or young person need. They will ensure that suitable provision/education settings are put in place to meet the assessed need, monitor the EHC plan and ensure that outcomes are being met in partnership with the education setting. 13

LEWISHAM SERVICES FOR CHILDREN & FAMILIES: AUGUST 2018 AREA SERVICE CONTACT Disabled Children s Social Care Service The Social Work team operates across the full spectrum of social work interventions including child protection, Children in Need, Looked After Children and Transition. The Disabled Children s Social Care Service is responsible for assessing the needs of disabled children and young people from 0 18 years (last day of the 17th birthday) who live within the London Borough of Lewisham. Social care referrals can come from parents/ carers, schools, other local authorities, health visitors, and health professionals. We encourage and support the use of inclusive and universal activities and services and support families to be informed and empowered to be more resilient and independent in meeting the needs of their children and young people with disabilities. LEWISHAM SERVICES FOR CHILDREN & FAMILIES: AUGUST 2018 AREA SERVICE CONTACT Address and contact: Noah's Ark Children's Venture Short-stay residential breaks in Macaroni Wood, in the Cotswolds just 2-3 hours from SE London. We are based in 18 acres of woodland and offer a range of activities both on and off site. Support is focused on three evidence-based delivery approaches (Triple P Level 4, Solution Focused Brief Therapy and Team Parenting), which enable children and families to build resilience, set achievable goals and develop positive relationships. Please include your child's name, date of birth, address and telephone number. Some parents provide all of the education themselves and some provide it in cooperation with other parents and neighbours or tutors. Some use materials and ideas from outside agencies such as Education Otherwise and some use correspondence courses. Elective Home Education Children between five and 16 If you are withdrawing your child from the school that he or she currently attends and if he or she is of statutory school age (between five and 16), you must write to the head teacher of your child s school. You must request that your child be removed from the school roll and state that you are fulfilling your legal duty to provide their education other than at school. It provides intensive outreach family therapy for young people and their families where the young person (aged 10 17) has persistent and significant conduct problems at home, school or in the community Eligible young people would be at risk of offending or offending; have problems with substance misuse; and have anti-social behaviour.
