  2. The Scale of the Problem. What is the Digital Weight Management Programme (DWMP)? Nationally 24.2% of adults are obese. Increased risk of diabetes, heart disease and certain cancers. 12 week online behavioural and lifestyle programme for patients with obesity and diabetes or hypertension. Reduced life expectancy and healthy life expectancy How can we improve patient engagement? Can we reach a different group of patients? Digital weight Management programmes Weight management programmes are typically accessed by women in ther 40- 60s. Typically accessed by younger (working age) people. The goal is to reach people earlier in their health journey before they make an appointment with professional. Maintain comparable results to face to face weight management services. a health care

  3. How did we find and contact our patients? Intervention Patients were sent a short, text message explaining why they had been contacted, benefits of a heathy weight and offering a simple intervention Patient Population Text message A patient search was undertaken across the PCN. This identified patients with a BMI over 30 kg/m2 within the last 2 years Personal Encouraging Included social norms Behaviourally informed : Evidence from behavioural science shows that written content can substantially impact behavioural outcome. Only a simple action required from the patient Specific exclusions applied so only appropriate patients contacted

  4. Increasing engagement with Digital Weight Management Programme 306 referrals and counting Patient search Group Text Referral form New referrals independent of patient seeking healthcare. Patient replies yes or stop Digital weight management programme referral completed by non-clinical staff To identify obese adults with specific comorbidity. At time of writing: 1897 texts sent 16% positive response Inclusions; age 18-30, BMI 30-50 within 2 years, DM or hypertension Exclusions; >80, house bound, learning disability, pregnant, eating disorder, recent weight management referral, cancer, mod/severe frailty, schizophrenia, bariatric surgery

  5. How much did the project cost? Minimal use of admin time only. Only existing service and infrastructure used. Was there negative feedback from the patients? <5 obese patient would have preferred not to be contacted. <5 patients were incorrectly identified as obese (initial search criteria error). Frequently Asked Questions 0.2% complaint rate Did it lead to an unmanageable additional workload for admin staff? No. Local solutions created. E.g. Two practices sent messages in batches.
