Empowering Communities: Community Health Workers in Action
Community Health Workers (CHWs) play a crucial role in connecting communities with healthcare services and resources. They provide health education, conduct home visits, assist with enrollment in programs, and advocate for community improvements. CHWs have been shown to improve health outcomes and reduce costs by promoting preventive services and managing chronic conditions. Funding for CHWs can come from grants, public and private sources, as well as through Medicaid.
