Person-Centered Planning is for Everyone! Here s What, Why, Who, and How Presented by Person Centered Planning Partners A program of Community 4 Each, Inc., a 501(c)(3) nonprofit organization
The Band-Aid Effect Band-Aid is a brand that has become a catch-all phrase for any adhesive bandage. There s only one Band-Aid. Person-Centered Planning is a real thing, but is being used as a phrase to describe any transitional planning process. Person-Centered Approaches or Thinking refers to specific ways that organizations can change the way they operate by adopting tools that generate conversation, account for the person s personality, improve lives by looking at staff compatibility, methods of communication, and conflict resolution.
Elements of the real thing Begins with the person s own dream, does not limit that vision based on systemic, physical or functional limitations. Honors the dream: even the impossible. Develops the goals based on the dream, shaping them into the positive and possible. Identifies and fosters the development of a sustainable Circle of Support; beyond the systems.
Elements of the real thing people committed to each other the absence of a hierarchy among participants attentive listeners without distraction or agendas other than the process skilled facilitators not bound by systems limitations
Why its important What s your morning routine?
Why its important You ve just moved into a group home. Here s your new morning routine. 6:30am staff or alarm will wake you make sure you respond with good morning to everyone who says it to you 6:45am you shower (no baths) if you don t shower in the morning, you may get dressed then sit in your room quietly until 7:00 7:00am breakfast house has choice of 2 or 3 items, does not include coffee or tea because it s not a healthy option and it s hot 7:20am meds for all 7:30am van arrives. Put on your coat & shoes, go sit in your seat.
Why its important How are you feeling? How might this affect you?
Why its important We are the stewards of our own humanity and for those who cannot stand for their own, on their own. Quality of life Greater clarity & focus for school & support systems More successful outcomes Sustainability
Who its for Expecting a transition in the future (2-5 No specific transition, but seeking a new way to introduce themselves and communicate what s important to and important for them. years). Examples: transitioning out of school or supported environments (group home, mental health units/psychiatric care) changing home or employment situations, marriage/divorce, aging into supported living.

How it becomes part of an IEP or ISP Individuals and families are taking their plans and revamping their IEP and ISP goals to reflect their plan s positive and possible goals, and to work toward skill building and resource acquisitions that are true to the real future they anticipate. School team are participating and finding the plans very helpful in their decision-making process. By having clearer pictures of the person s identity (visa vie the gifts), goals, natural supports and life outside of school hours, they can make more informed decisions and strengthen outcomes that apply to all environments. DDS and other systems folks are participating. After a recent plan was complete, the DDS representative said I don t understand why this isn t being done right now for every student in transition!
