Mental Health Focused Fellowship Project Presentation DR NKIRU OLUWATOSIN 1/5/2024
Introduction My name is Dr Nkiru Oluwatosin Salaried GP Munro Medical Centre Spalding Trained Mental Health First Aider Certified in the Fundamentals of Neuroscience Special interest in Mental Health
Commencement of Fellowship Completed my GP training and obtained my CCT January 8th2020. Commenced the new to practice fellowship programme January 2021. I had 2 babys during my programme so went on maternity leave twice. Will be concluding completely August 2024
Intended Personal Goal for Fellowship Programme I commenced my fellowship with a desire to get involved with leadership in mental health with an aim to contribute towards overall patient care and population wide improvement in mental wellbeing.
The Journey 1st6 months Attending mental health conferences, discussions with mentors Further time spent completing courses, e.g the mental health first aid course MHFA England, fundamentals of neuroscience Harvard online Made connections with relevant people, teams and staff required to achieve my desired mental health goal impact within my PCN and county Got appointed to be the PCN Primary Care Mental Health Champion for Spalding, responsible for facilitating the implementation of the National Community Mental Health Framework within my PCN
Support from Mentors I had 3 mentors through my programme All 3 have played very key valid roles through my experience The first mentor supported me in getting settled and comfortable with the concept of the fellowship programme and begin to formulate my planned outcomes. The Second mentor who was a GP active in mental health helped me navigate my desired outcomes and upon realizing my goals was able to introduce me to the relevant teams and network where I could actualize them. My last mentor is now performing the role of helping me consolidate on all I ve accomplished to make sure I end well. I truly appreciate all the mentors I have had, as I do not think I would have been able to achieve my goals without them. They have been extremely helpful and supportive.
Mental Health Project Embarked on A Project to Facilitate The Implementation and Embedding of the goals of the National Community Mental Health Transformation Programme within Primary Care including Holistic Assessments, Patient centered trauma informed care, well integrated care pathways, effective utiltisation of community-based assets and patient empowerment for self care. with a focus on Holistic assessments and Patient Psychoeducation for better patient outcomes and effective utilisation of Community Resources Here s a short video describing the National Community Mental Health Transformation Programme Team Dr Nkiru Oluwatosin (GP Munro, PCN Mental Health Champion Spalding) Tanya Khan (Senior Mental Health Practitioner Munro Spalding)
Project Features and Accomplishments Main strategy was EDUCATION Measurement was by a QUALITY IMPROVEMENT ACTIVITY including a 2 - Cycle audit and Obtaining of feedback from colleagues and patients showing impact.
Project Features and Accomplishments EDUCATION Education of colleagues on better ways for holistic patient centered clinical assessments prioritizing patient s ideas, concerns, expectations, unique beliefs, culture, or religion/faith and incorporating these alongside clinical assessments in over all care. Education of colleagues on available community resources and and available wider community mental health support teams e.g community connectors, wellbeing coaches etc to assist with increased effective signposting and utilization of community resources. Psychoeducation of patients for promotion of mental wellbeing and empowerment for self care. Adults and children. KEY MESSAGES -1. Patient Centered Holistic assessments 2. Integrated Care Pathways 3. Promoting The evidence based 5 ways to wellbeing public mental health message (known population wide positive impact on mental health.) 4. Locations, contacts, and links for community resources and supportive mental health teams made available for clinical colleagues
Key messages : The 5 Ways to Wellbeing nef s subsequent report, Five Ways to Wellbeing: The evidence, presented five key messages from the review, described in a simple, memorable and appealing format (see Box 1).19 The messages were developed to reflect kinds of behaviour that people can undertake, and which academic evidence suggests may lead to improvements in their mental health and wellbeing. The Five Ways to Wellbeing was developed based on evidence relating to individuals behaviour. If individuals change their behaviour so as to incorporate more Five- Ways-type activities into their day-to-day lives, the empirical evidence suggests that their subjective wellbeing should improve. Project Features and Accomplishments Education for colleagues achieved via mental health presentations, peer to peer colleague conversations, provision of resources and pointers towards needed information in clinical consulting rooms including, posters with information, relevant links and QR codes Education of patients achieved via psychoeducation during clinical consultations, posters in patient waiting rooms and provision of resources with information and links to relevant community resources e.g automated texts, posters in patient waiting rooms Development of a mental health template to guide assessments and psychoeducation especially for the mental health practitioners Planned live mental health workshops within secondary schools in the PCN Clinical Audit Obtaining of feedback from colleagues and patients to acknowledge, reflect and action as necessary
Project Features and Accomplishments 1. Mental Health Presentation given to GP colleagues and other allied health professionals (January 2024) 2. Presentation to GP trainees Boston (May 2024) 3. Poster placements in clinical consulting rooms, waiting rooms across practices in PCN. 4. Development of mental health template within systemone incorporating main aspects of holistic assessments and psychoeducation. (Completed and in use by the mental health practitioner.) 2024. 5. Quality Improvement audit done to assess provision of holistic care, psychoeducation and effective utilization of community resources. (1stcycle done January 2024, Second Cycle May 2024) 6. Live mental health workshop for secondary school children in Spalding. (Preventative as 50% of mental health problems are established by age 14 and 75% by age 24. Project facilitator contacted, to be kicked off once funding concluded.

Impact of Project Example of Feedback from Colleagues following Mental Health Presentation Given
Overall Feedback and Success of Project Impact on colleagues As noted from feedback and also conversations, colleagues feel more aware of community resources available and are also now more holistic in their approach meeting the goals of the national community mental health transformation framework. Impact on Patients According to our mental health practitioner, her need for prescribing medications has gone down, as well as the fact that patients are saying they feel more empowered to take steps towards their improved mental wellbeing because they are now more informed of what to do, where they can go and who can help. (We are currently putting together written documented evidence from our patients which we will be presenting at our next clinical governance meeting of impact made by project so far on patients.) The second cycle of the clinical audit will further help us assess impact on our overall practice towards holistic assessments and patient psychoeducation

Personal Reflections The New to practice fellowship Programme is one I am grateful I undertook. It gave me the opportunity to pursue my interests in mental health from a leadership perspective. It enabled me understand the NHS framework better from a PCN perspective and enabled me play a greater role in impacting positively on patient care by allowing me the opportunity to create and support systems in place for greater population wide mental health impact. I have derived a greater sense of job satisfaction and career fulfillment.
