Self-Evaluation form for HCP Extract from the AMEQUIST Evaluation ToolBox_030222.pdf and from the AMEQUIST Evaluation Manual_030222 .pdf The self-evaluation will be submitted via the on-line tool. The self-evaluation form in excel (HCP-Self-Evaluation-form-toolbox.xlsx) will facilitate the previous collection of information. The excel file (HCP-Self-Evaluation-form-toolbox.xlsx) includes 2 sheet, which contains 3 main sections (A, B and C) A total of 24 operational criteria with 64 measurable elements classified in 7 areas have been developed
Self-Evaluation form for HCP- Section A Section A includes specific questions for the identification of the HCP team, the area of expertise, services provided and activity. Information is also requested about the barriers encountered for the involvement of the HCP in the activities of the Network, leaving a final section for the HCP representative to make any comment he/she deems appropriate.
Self-Evaluation form for HCP - Section B The section B contains all the criteria and measurable elements with the scoring options. Please refer to the scoring guidelines (scoring-guidelines.pdf) which provide specific guidance for the scoring process. In the "Comments" section, the HCP representative can enter the explanations related to the requirements of each ME and those specific and differential aspects of the HCP that must be considered in the scoring. Self-Evaluation form for HCP - Section C The section, "Evidence", is intended to incorporate the references that show compliance with the ME. This evidence can adopt different ways: - Documents that describe how the HCP carries out different processes, activities, flyers, etc. - List of indicators, guidelines and protocols, patient associations, etc. In both cases you can attach the document or the link that allows the IEB evaluator to access the document.
