  3. AANJIBIMAADIZING FOCUS AREA Aanjibimaadizing provides services to support a portion of the housing continuum. We support affordable rentals and affordable home ownership in emergency situations. Aanjibimaadizing supports this because housing is a critical part of family stabilization.

  4. ZAKAB There are 38 units across all districts Transitional housing This is safe and sober housing Case management is required prior to, and during, living there Clients must sign housing requirements before move in

ZAKAB SUCCESS STORIES 16 children reunified with parents 7 babies born 238 referrals for outside services 32 of 38 units are currently occupied (one is an office and the remaining are being prepped for move ins) All families have increased their financial health All have increased their natural supports and increased family stability

TYPE OF ASSISTANCE ELIGIBILITY REQUIREMENTS One-time financial assistance may include: At imminent risk of homelessness Low income Employed, likely to be employed with sustainable income within 3 months, or connected to other income (SSI, etc.) that will likely be sustainable within 3 months Exhausted other resources, and no remaining resources to resolve the crisis Likely to stabilize with limited assistance (less than 3 months) Projected income exceeds expenses Application/Intake Form Release of Information Documents FHPAP HMIS Forms Copy of housing plan/case plan Other forms as relevant to client Income verification Copy of lease Landlord information forms Rent calculation form Direct assistance financial forms Other Cannot receive this assistance from more than one source.   12. TYPE OF ASSISTANCE ELIGIBILITY REQUIREMENTS The Homeowners Assistance Fund is federal funding that can provide full or partial payment assistance to eligible homeowners unable to make full delinquent mortgage payments and provide funds to resolve delinquent payments for utility services, excluding phone and internet, due to a continuing financial hardship associated with the Coronavirus pandemic. Personal identification number from a state-issued ID, driver s license, passport, ITIN, Tribal ID or other form of ID. Statement of current inability to resume mortgage payments due to unemployment, underemployment or other continuing hardship. Total amount of each type of assistance (mortgage payment, mortgage arrears, utilities, home energy costs, utility arrears) needed. Number of months of mortgage payments and number of months of utility cost with documentation of amounts owed. Gross household income of all adults and number of individuals in the household for the past 2 months, or a tax return for 2020. Each participant will be required to provide income information as well as sign an attestation stating that the income provided is the only income for that individual. Copy of default invoice. (overdue bill, foreclosure notice, etc.) W-9. (Mortgage Lender and/or Utility Provider) Additional information may be requested. Be a homeowner Be a Mille Lacs Band Member, Mille Lacs Band descendant, or have a Mille Lacs Band member living in their home Live in the State of Minnesota if MLBO Descendant Homeowner is at least one installment payment in arrears on one or more utilities Assistance sufficient to resolve the delinquency is not available from other utility assistance programs and, without HAF assistance, the homeowner is likely to lose services. There is an amount maximum for this funding. Only a maximum of 12 months of assistance can be covered. HAF Homeowners Assistance Fund

TYPE OF ASSISTANCE ELIGIBILITY REQUIREMENTS One-time financial assistance may include: Must owe rent or rental utility payments on a residential dwelling Be a Mille Lacs Band Member, MLBO 1st Descendant, or have a child living in the house that is an MLBO member Live in the state of Minnesota and not receiving rental assistance from another local agency Have children living in the home who are in Pre-K through Grade 12 school and are experiencing ONE of the following: Living in overcrowded conditions in their current housing using the HUD definition Paying more than 50% of their income for rent Lacking a fixed, regular, and adequate nighttime residence Are at risk of eviction, loss of power, or loss of heat. Must apply to be an Aanjibimaadizing client. Rental Assistance 1st/Last rent Deposit The Local Homeless Prevention Assistance application must be done in person or over phone with a Case Manager or Facilitator after the Aanjibimaadizing application is complete. Utility assistance (including past due) Receipt of this funding requires participation in housing stability support through case management. Other material and financial assistance (only upon approval of Grantee) LHPA Local Homeless Prevention Aid

OTHER SUCCESS STORIES Aanji has assisted more than 4982 families with housing barriers A person was evicted, found place on his own, started a job, we were able to assist with deposit Helped approximately 100 elders in the community to pay utility costs last winter Had an elder who was experiencing significant illness and as a result fell behind in house payment and utilities. We were able to assist with making these payments to keep her housed in a time of extreme need Total funds distributed as of 9/30/23: $6,009,089 Many have received furniture and/or home supplies. These were donated to the program.
