Child Outcome Summary Form Understanding COSFs
Outcomes 1. Positive Social Emotional Skills (including social relationships) 2. Acquisition and use of knowledge and skills (including early language/communication [and early literacy]) 3. Use of appropriate behaviors to meet their needs
EDHI Minnesota requires a report of each child s hearing status to be collected in the COSF, as well. This report is complete by answering up to 16 questions, depending on the level of impairment.
Age Expected items: skills are demonstrated by the child that are typical for children of the same age. Age expected with qualitative concern are skills that are typical for same age but the functionality of the skill is concerning. Immediate Foundational Skill items: (1-24% below age) skills prior to age expected skills but do not indicate a significant delay Foundational Skill items: (25% below age also -1.5 SD) skills demonstrated are well below age expectations
When to Complete COSF Entrance Exit: Teams should complete the exit Child Outcomes Summary process as close as possible to when the child receives his or her last service. If moved in/out To provide data for the federal reporting requirements, the process must be completed when the child enters the program and again at program exit. The rating reflects the child s functioning at each of these time points and should be determined as close to the actual entry and exit as possible. The comparison of exit to entry ratings provides information about the child s progress.
