2019-2020 Senior Exit Surveys
In the Senior Exit Survey process, 12th graders in the School District of Philadelphia can access the survey link via the Student Portal. By following specific steps within the Message Center, students can navigate to the Surveys section and complete the Senior Exit Survey on Survey Monkey. This process is specifically for 12th-grade students during the year 2020 and is integral for gathering feedback. The Educational Options Program (EOP) students have a different survey process administered by counselors.

When 12thgraders log into the Student Portal (https://sso.philasd.org), they are redirected to Infinite Campus. Their default page is their schedule and assignments for the day. 2 School District of Philadelphia Office of Research and Evaluation
Students must click the Message Center menu item on the left 3 School District of Philadelphia Office of Research and Evaluation
Within Message Center, students will see a list of icons (see Student Launchpad below) 4 School District of Philadelphia Office of Research and Evaluation
Students must then click the Surveys icon. 5 School District of Philadelphia Office of Research and Evaluation
Senior Exit Survey: The Senior Exit Survey link will redirect students to Survey Monkey to complete the Senior Exit Survey. *Will only be available for students in grade 12, and actual date will be during 2020 **Students graduating from the Educational Options Program (EOP) will not see the Senior Exit Survey.
