  2. Session Aims To outline the proposals for the new planning pathways for children requiring additional support with learning To provide interim guidance for those children requiring significantly enhanced support for key transitions (Pre-Nursery to Nursery, Nursery to P1 and P7 to S1)

  3. Reasons for Change Presumption of Mainstream Development of a needs led system Current systems no longer fit for purpose Support to build capacity in mainstream schools Review of outreach services was required Review of devolved ASN budgets required

Proposed New Model CoS Level 1 Curricular and in-class planning to meet learner s needs CoS Level 2 Inclusion & Wellbeing Forum (Education resources) CoS Level 3 Multi-agency support (e.g. MHMWB Screening group) CoS Level 4 Education Placement Group (WL special schools / units) Multi-Agency SORG (external special provision)

Interim Transition arrangements 2015-2016 New planning framework to facilitate multi-agency assessment Aim is for this to be completed by multi-agency partners, parents and young people in a specific transition planning meeting hosted by current school / placement Only one (joint) report required including a full profile of current support package Where partners cannot attend, they will be asked to submit their assessment using the universal format The catchment receiving school will attend these meetings to provide information about what can be offered currently in that setting Focus is on need and on what would be needed within a mainstream setting to facilitate a successful transition

  6. Interim Transition arrangements 2015-2016 Meetings to take place between now and the end of January 2016 as part of inclusive transition planning. In circumstances where significant concerns are highlighted, paperwork to be submitted to the Inclusion & Wellbeing Forum by 12th February. After this, invitations to attend the Forum will be arranged. All information will be considered by the Inclusion & Wellbeing Forum and decisions regarding additional support / placement will be confirmed by 30th April. This is an interim arrangement only which will inform the strategic plan. Going forwards, it is expected that pupils requiring significant transition planning will be known to the Inclusion & Wellbeing Forum in advance and that needs can be planned for at earlier stages.
