  2. BP/AR 6158 - Existing BP/AR 6158 Independent Study Program - Physical Education A. Criteria and Eligibility 1. CVUSD approved program 2. I.S. agency submits application prior to student may apply 3. I.S. agency submits CVUSD Hold Harmless Agreement - I.S. PE 4. For approval the student must be performing: a. At an advanced level and b. Be individually ranked at the i. National ii. Olympic or iii. Elite level Travel teams, club teams, etc. do not qualify for I.S. PE 5. Grades 7-12, 2.0 GPA, and earned PE credit w/ C 2 previous periods 6. Time in activity must equal or exceed 10 hours/week 7. I.S. PE must continue for complete semester or year continued...

A schedule and location of activity b. Student s course schedule c. Summary of student s grades for one year d. Statement of learning objectives and competencies to be met e. Method of evaluation of the objectives f. Duration of the contract g. Specifics as to reporting student progress & attendance h. Signature of professional certified instructor i. Signature of student and parent j. Signature of principal (or designee is our practice) k. Signatures of PE DC who will act as IS PE supervisor l. Personal insurance provider w/group & ID number m. Instructor s resume 9. I.S. PE is graded on a pass/fail grading system 10. State-mandated tests must be completed: Grades (5), 7 & 9
