Take prerequisites classes If you have credits from another college Request official transcripts to be sent to ACC and evaluated; once approved they will show up on your ACC transcript 5.   16. APPLICATION CHECKLIST APPLICATION CHECKLIST Information Session Confirmation Email (required). ACC Transcript (required). Upload an ACC transcript that shows your grade for each of the following classes: ENG 121, PSY 235, BIO 201, HPR 108 or HWE 100. You must have a minimum combined GPA of 3.0 (for those four classes) and each course must have a grade of C or higher. All prerequisite BIO courses must be completed within seven years of the application. Health Care Employment (optional). Any employment within the last 5 years that had direct interaction with patients in any way. Upload a completed Employment Verification Letter for each employer. If you have none then check not applicable . Health Care National Certification or State License (optional). Upload a copy of each. Please do not include your CNA license in this category. Examples might be EMT or LPN or MA. If you have none then check not applicable . HESI Exam (required). You need to take the A2 also called the RN Admission Assessment. Upload your test summary that shows at least 70% English Composite Score and at least 70% Math Score. You may take the exam a maximum of two times in the 12 months prior to submitting your application. Only one summary page is allowed; you cannot combine different tests. Connor-Davidson Resilience Scale. CNA or LPN License (required). Upload a copy of your license or you may submit a letter from an instructor that you have taken the class and are waiting to take the State Board Exam. Volunteer Hours (optional). Any volunteer hours of at least 40 hours to a maximum of 120 hours within 2 years. Upload the Verification Form. Eligible to take MAT103. This is met by taking BIO201. MAT103 must be taken within one year of starting the ADN program OR during the first semester of nursing classes.

HEALTH CARE EMPLOYMENT HEALTH CARE EMPLOYMENT VERIFICATION VERIFICATION Print as many copies of the blank form as you need If you have no hours, this is not a requirement Up to 10 points possible Include CNA and/or other health care employment with direct patient-care contact within the past 5 years 4000 hours of CNA or other healthcare employment is worth up to 10 points. This must be validated by a supervisor or HR Director

Two to choose from: Prometrics (in- person) or ProctorU (remote) If accommodations are required please contact the vendor as this is a 6 week process and ACC has no input or control over this process

IMMUNIZATIONS (UPON IMMUNIZATIONS (UPON ACCEPTANCE) ACCEPTANCE) Immunizations must be complete the week prior to starting the program Spring Start applications must include an influenza shot Fall Starts will be advised to get influenza shots in October If titers are used, you must provide lab records Lab records will be required if offered admissions and titers are used in place of immunizations COVID-19 vaccine is required upon acceptance

Each course must be a C or better. Multiply grade value (A=4, B=3, C=2) by number of credits, for each class. Add together then divide by either 11 or 13 depending on which Nutrition class you took. Health Care Employment: 10 points possible HESI Science Composite Score: 5 points possible Health Care Certifications: 2 points possible CNA taken at ACC: 1 point possible Volunteerism: 3 points possible Connor-Davidson Resiliency Scale: 20 points possible Average of HESI English and Math: 55 points possible Must score at least 70% in both areas English and Math scores averaged together for total Total Score is Candidate Criteria Points + HESI Average: 100 Total Points Possible

BACKGROUND & DRUG BACKGROUND & DRUG SCREEN SCREEN Done after acceptance to the program When you pay for your background check, you will also be pre-paying for a drug screen The random drug screen will happen at some point between acceptance and your first clinical assignment A Medical Marijuana card is not an eligible exemption

TYPICAL WEEK SCHEDULE TYPICAL WEEK SCHEDULE On campus or online 3-4 days per week, 8:30a-4p May include lecture classes 2 days per week, up to 3 clinical shifts per week and lab assignments Clinical shifts may be 8, 10 or 12 hours and may be days, evenings, or overnights, any day of the week Recommend you do not work during this program, or try to keep it to less than 20 hours per week Flexibility in work schedule is necessary Clinical schedules change twice each semester
