The story of Ms O. - 90+ year old - Lives alone - Housebound for the last couple of years - Cognitive decline - Falls risk - No service input
Transformational change (1 year on) Integrated OT service Dedicated therapist input in Enablement Single shared IT system Joint decision-making across the 3 teams
Lessons learnt 1 Staff and user consultations bottom-up process 100+ staff and users consulted Co-production approach in 1 Dundee ward
Lessons learnt - 2 Collection, collation and analysis of data Dispelling myths and agreeing on facts Analysis of client pathways (to identify improvement areas)
