SWOCOG FSS-Respite Care w/Individual
The family can add any respite provider 18 or older that does not live in the
home. It cannot be a custodial or non-custodial parent. Forms needed:
Provider information page
W9 form
Family waiver page
Request for Vouchers form
Once we have added the provider they remain on file from year to year.
Vouchers must be requested prior to the respite care occurring.
SWOCOG FSS Summer Camp
The family can use any summer camp but not all will take our vouchers. Forms needed:
W9 form
Request for Vouchers form
If the family wants to pay for the summer camp and be reimbursed, the family would
fill out the W9 form. If we are paying the camp directly, the provider fills out the W9
form. Once we have added the camp they remain on file from year to year so we do
have some summer camps on file. Vouchers must be requested prior to the camp
occurring.
SWOCOG FSS Ipads/Computers/Laptops
Ipads/computers/laptops can be approved for children 12 and older one time
every 3 years. Maximum amount that can be approved is $500.
Forms needed:
Verification of need form completed by a doctor or therapist justifying the
need for the item. Can’t be justified for school, educational need or service
required by IEP.
Price quote from micro center if you would like FSSP to issue a check up
front. Or another option is a quote from anyplace but this will be considered
a reimbursement. W9 will need to be on file for reimbursement with parents
information.
Please do not purchase until you hear it is first approved if going the
reimbursement route.
SWOCOG FSS Emergency Rent
FSSP can help out with up to the maximum amount of $200 one time every
other year for rent.
Forms Needed:
A letter from your landlord on letterhead stating you are behind, how much
you are behind by, who the check needs to be made payable to along with the
landlord signing and dating the letter.
The landlords w9
Send the letter and w9 to me for approval.
SWOCOG FSS Vehicle Repair
FSSP can help out one time every other year up to the maximum amount of
$200 for a vehicle repair.
Forms Needed:
A completed verification of need form completed by a doctor or therapist
justifying the vehicle repair.
Quote from garage where the work will be done.
W9 from the garage.
Once completed send me the completed verification of need form and price
quote all together.
SWOCOG FSS Learning Tablet
Maximum that can be approved is $150.
Forms Needed:
Completed verification of need form completed by a doctor or therapist
justifying the need for the items.
Price quote from Micro Center if you would like us to issue the check upfront.
If you would like family reimbursement the quote can be from anyplace. Once
you have the completed verification of need form and price quote send those
forms to me all together to work on approval.

Energy Or Cincinnati Water
Forms Needed:
A copy of the PAST due Duke Energy or Cincinnati Water Bill.
FSSP can help out with up to the maximum amount of $200 one time every
other year.

Examples: Bed, high chair, stroller, car seat, storage items, and safety items.
Maximum that can be approved is $150.
Forms Needed:

SWOCOG FSS Adaptive Equipment and
Home Modifications
FSS will consider any adaptive equipment or home modification. Forms needed:
Verification of Need form
Letter of medical necessity from a doctor or therapist that explains the
therapeutic value of the equipment or modification.
Does the family own or rent the home. Please write that on the front of the
verification of need form.
Quote from the vendor that includes shipping, tax, etc.
W9 form
Local vendors will take our vouchers but not online or catalog companies. We can
issue a check made out to the vendor as long as we have a W9 on file for them and
they are willing to take a check as a form of payment. If this will not work, we can
handle it as a reimbursement to the family with the family’s W9 form on file. All
requests have to be approved prior to making the purchase or starting the
modification.
SWOCOG FSS Training/Therapy
FSS will consider many types of therapy, training, and counseling as
long as it if offered in a non-educational setting. Tutoring is not
covered. Forms needed:
Request for Vouchers form if they are a provider that will
accept our vouchers.
If they do not accept our vouchers then the family can pay and
send in the receipt for reimbursement.
W9 completed by the family for reimbursement.
Once a family has been approved for a specific type of therapy,
we keep the documentation on file from year to year and they
just need to fill out a Request for Vouchers form each year to
obtain new vouchers. Vouchers must be requested prior to the
therapy, training, or counseling occurring.
SWOCOG FSS Incontinence Supplies
FSS will fund diapers, pullups, attends, wipes if the individual is
age 4 or older. Forms needed:
Request for Vouchers form
Completed verification of need form justified by a doctor or
therapist.
Kroger is the company that we have an account with that will
accept our vouchers. If the family wants to purchase diapers
elsewhere, we can process the request as a reimbursement to the
family with the family’s W9 form. All requests have to be approved
prior to making the purchase.
SWOCOG FSS Special Diets
FSS will consider nutritional shakes for example pediasure, ensure or
boost. Forms needed:
Verification of Need form
Letter of medical necessity from a doctor or therapist that
explains the therapeutic need and benefit.
Request for Vouchers form
Kroger will accept our vouchers for this as well. If the family wants to
purchase the special diet items elsewhere, we can process the request as
a reimbursement to the family with the family’s W9 form. All requests
have to be approved prior to making the purchase.
SWOCOG FSS Program provides assistance for respite care, summer camp, iPads/computers/laptops, emergency rent, and vehicle repair. Requirements, forms, and maximum amounts for each category are outlined for families seeking support.
