CDCS PLAN BUDGET CATEGORIES Personal Assistance Staffing Treatment and Training Therapies, Specialist Services Environmental Modifications Hard goods, transportation, chore services Self-Direction Activity FMS and Support planner fees, payroll taxes, PTO, etc. NOTE: Health and Safety plan requirement-either CSP or separate document

CDCS PLAN REQUESTS Must be allowable DHS guidelines as well as lead agency policy Must be approvable for the member A service/item may be approvable for one but not another Must be tied to assessed needs of member Ex: Ramp not approved if mobility not an assessed need All requests made by member need to be presented Support planner may assist in guiding requests but cannot deny

  5. WHY CHOOSE CONSUMER-DIRECTED (CDCS)? Highly individualized plans-person-centered Most flexibility in care Able to keep staff with higher rates of pay than agency or traditional waiver Still able to use services similar to traditional (MOW, PERS, etc.) Paid spouse is an option More control over other providers

  6. SUPPORT PLANNER ROLES & RESP. Consultant type role Work with all parties (members, Care cord., FMS, vendors) Assist with problem-solving, resources, trouble-shooting Write the CDCS Consumer Support plan w. member Ensure assessed needs are addressed Understand allowables vs. unallowables Understand lead-agency specific nuances Help keep the ball rolling Can assist the member in identifying a FMS Understand the SEIU union rulings and implementations Make revisions to the CDCS plan throughout the year as needed

  7. WORKING TOGETHER W. CARE COORDINATORS CC typically reach out to support planners after MN Choices and member has decided to go CDCS CC would provide support planner w. MN Choices summary and case mix/budget amount Support planner would then schedule meeting w. member Support planner will write CDCS plan w. member and ensure FMS is hired Support planner submits plan to CC w. signatures for approval along with any addt l documentation (home mod bids,etc.) Signatures must be sent to CC prior to plan going to review

  8. WORKING WITH FMSS Pay into payroll tax, Soc. Sec., work comp & unemployment Monthly reporting Exception Reporting Reimbursements to members or their staff for mileage Pay vendors

  9. WHAT ABOUT CHANGES/REVISIONS? Changes to CDCS plans can occur throughout year Examples: COLA increases, new request, vendor changes, increase in wage request, transportation changes, etc. Support planner submits request to CC Support planner ensures that the math is correct For example: If a member wants to raise a wage for a staff and all funds are currently allocated-another category will have to be decrease. Support planner verifies with FMS what funds are available to know where to make change

  10. COMMON MISNOMERS FOR CDCS- EW You can t use traditional services while on CDCS... You can use EW budget for home repairs-i.e. new roof Technical assistance doesn t count on EW Your budget stays the same every year You will not qualify if you already have insurance

  11. BARRIERS TO SUCCESS W. CDCS FOR EW EVV (however also on traditional) Finding Staff Home may be issue that cannot be fixed w. CDCS dollars Members may request items that are unapproveable Ex: Member may request roof repair to stay in home Ex: Member may request comfort item w. no assessed need Being an employer has responsibility and rules
