Evidence to Include Pupil Views Parent/Carer Views Outside agency information Education EP/PSS/CAT/PDSS/SS (as appropriate) Attendance Record School/Setting Tracking Information School/Setting Attainment Information Evidence of Assess, Plan, Do, Review cycle, e.g. Target Plans, review meeting notes.
Funding Currently funding is being granted based on CRISP profile using the CRISP calculator Funding is granted in half termly amounts e.g. Spring 2, Summer 1, Summer 2 If needs are medical and therefore unlikely to change over the year funding may be granted for the year. SEND Support Plans will need to be reviewed and resubmitted if funding needs to continue.
