  1. How to CORRECTLY fill out the Science Fair forms Jennie L. Borders

  2. Common Sense You must write in pen when filling out the forms. Black ink is preferred, but blue ink is also acceptable. NO PENCIL!! No colorful ink like pink, red, green, sparkly, etc. EVERY group needs a form 1, 1A, 3, and research plan. Each PERSON needs a form 1B. Depending on your topic, you may need other forms that I will provide. You may also need prior approval through the science fair board before you start.

  3. Form 1 Checklist for Adult Sponsor Write your first and last names LEGIBLY! Write your title LEGIBLY! Check the following boxes! You need to determine (or ask me) if you need to check any of the following boxes.

Form 3 Risk Assessment Form All Members first and last names Write your title List any hazards like chemicals, bacteria, dangerous procedures, etc. Identify risks like contamination, possible injuries, etc. List safety precautions like wearing safety glasses, wearing gloves, working in the lab with a teacher, etc.

  10. Form 3 Risk Assessment Form (cont) Write about how you will dispose of your project. For example, wash down the drain, throw away, dilute, hazardous waste container, etc. Flinn Scientific Any other source you used for your project. For example, science buddies I will fill out this box! Your parents do not fill it out!

  11. Conclusion In conclusion, FILL THE FORMS OUT CORRECTLY THE FIRST TIME. I DO NOT LIKE READING THESE OVER AND OVER AGAIN!!!
