The Learning Community for Person
Centered Practices

Envisions a world where all people have positive control
over the lives they have chosen for themselves. Our
efforts focus on people who have lost or may lose
positive control because of society's response to the
presence of a disability or other conditions.

LifeCourse Framework Core Belief

All people have the right to live, love, work, play and
pursue their life aspirations in their community.

What Really Matters?

Where do you want to go?
(Trajectory)
What is important to you?
(Person Centered Thinking)
How are you supported along the Journey?
(Person Centered
Thinking and Integrated Support Star)
Actions we take to achieve what really matters!

All levels:
Systems level, Communities, Organizations, Family, Individual

What Really Matters?

How can this question be applied at all levels and all people?
How do LifeCourse and Person Centered Thinking fit in
this pursuit?

4+1?’s

What have I tried with integrating my thinking and use of
PCT and Life Course – key area is

What have I learned about….?
What am I pleased about...?
What am I concerned about?
What are my next steps?

What have I tried with integrating my
thinking and use of LC and PCT?

Personal First!!! PCT on my journey with cancer & LC
with my son with disabilities – setting the trajectory and
integrated supports star.
Integrating Life Course into work with PCT Coaches
Training new Support Coordinators
Internal community of practice for LC
Integrating into the state ISP Guide
Presented at Virginia and TLC Gathering

What have I Learned?

Missing link to achieve what really matters at all levels
Both are brilliant, needed and help all people achieve
control, find hope in their lives.
Same language is used by people beginning to use both
concepts --- making it a habit. You see it playing out in
people’s words.
Same struggles experienced in rolling out/launching
among service professionals
Need to be able to articulate clearly how they skills and
framework compliment and work together. Different and
equally powerful.
SC’s need to know the philosophy, values and principles
of Life Course and Person Centered Thinking

Learned…

The goal is achieve person centered supports and help
people achieve their vision of a good life
Life Course adds another dimension to the conversations
and vision of what is possible
Teachers are ‘eh about the portfolio…but that is the
same as my experience with one page profile

Pleased about

LifeCourse deployment feels doable, energizing, and lots
of AMAZING stories very quickly – THE BUZZ!!!
Intuitive
Guides the work of the Support Coordinator
My son is clear about his vision for a good life
Life Course is trained in minimal time, graphics are very
helpful, information is easily accessible, knowing this is a
big national effort.
National effort – lots of partners in this work

Concerned About…

Pressure Ben puts on himself to achieve his vision of a
good life – working to learn it is a work in progress and
OKAY to change your mind.
I am doing what works to training new SC’s – integrating
and blending of PCT and LC. Others may not have this
opportunity – how do we make this easier to do?
Soooo many other things to do (personal frustration)

Next Steps

Collaborate with all of you!
Keep revising, learning
Practice taking small steps and feeling good about small
sustainable changes.
