Music Therapy Interventions
 
Based on an individual’s assessment and treatment plan
Receptive and/or active use of music-based protocols
Person-preferred music
Live music to be adapted in the moment
Instrumental, singing, movement, structured listening, visualization,
improvisation, songwriting, lyric analysis, adapted learning
Structured verbal dialogue, counseling, psychoeducation
May include family members based on the member’s preferences
 
 
 
MSHO - Music Therapy Benefit
 
Primary Music Therapist assigned and follows member’s care
12 sessions per calendar year
Frequency of visits based on Care Coordinator referral & member preference
In-person and telehealth options based on location & COVID-19
 
Candidates include MSHO Nursing Home Members:
diagnosed with Dementia, Alzheimer’s Disease
demonstrate signs of depression or low mood
who are at higher risk of isolation
who require additional mental health support
*no musical experience required
 
Enrollment in Services
 
1.
Care Coordinators
a.
Identify members who qualify for the music
therapy benefit
b.
Send the 
Music Therapy Referral Form
 to
referrals@alliancemusictherapy.com
c.
Notify Facility Contact of the Music Therapy referral
& that AMT will be contacting them for set up
2.
Alliance Music Therapy
a.
Confirms receipt of referral via email
b.
Coordinates services with facility site & assigns
music therapist to member (approximately 1-2
weeks)
3.
Member is ready to begin music therapy
 
Provider will:
Set up a session day & time with the
member or facility staff
Establish emergency contact &
protocols
Understand member preferences &
designate device for virtual sessions
Ensure requirements met
Stable wifi, ethernet connection
Tablet or computer with email access
