Good afternoon everyone I am Chiara Martini, a PhD student from the University of Genoa. Within my PhD project I started a collaboration with the Galliera Hospital in Genoa and here I will present an idea they have on a possible EU PCP project. The Idea is to build a semi-hospitalized facility for just discharged elderly patient.

Create a semi-hospital facility within or near hospitals to provide a homely atmosphere for discharged elderly patients. This facility aims to reduce rehospitalization risk, lower hospitalization costs, and improve overall quality of life for patients and caregivers. Expertise in architecture, engineering, and clinical measurements is required to design a comfortable environment with integrated smart systems for early health risk prediction.

MOTIVATION ~8% of hospitalized patients are potentially dischargeable but they end up staying in the hospital for different reasons [Lenzi et al. 2014 BMC Health Services Research] Inefficient occupation of hospital beds, increases hospital costs (~1000 /day) Frailty index of elderly increases after 12 days of hospitalization
