Sample 2021 Quarterly Board of Pension Form Template
#1 Complete the Date, Church Name (No Abbreviations) AND Full Mailing Address.
#2 Please ensure that your Treasurer AND your Pastor / IPL sign the form AND add a contact telephone number
#3 Please complete: Year Reporting Which Quarter Number of Active Members $ Amount for each month Add up each month One form per quarter Rate change for 2021, $1.75. Please make sure the calculation is correct Payments are due by 10th of the month following the end of the quarter
#4 Please add in: Check Number Check Date Check $ Amount
#5 Please also ensure that you provide an email address, in case we have questions
#6 Please ether MAIL your hard copy Assessment Form (and Check) to: UFMCC Board of Pensions PO Box 9917 Wichita Falls TX 76308 OR Email it to: BOP@MCCchurch.net Please DO NOT Staple the Check to the Form Payments are due by 10th of the month, following the end of the quarter
