Session Outcomes Complete the Plan stage of the DaSy Dynamic Impact approach to identify and develop SMART long-term outcomes, determine checkpoints, and identify the action steps needed to achieve outcomes. 2
Plan Stage Identify one or more long term outcome(s) Written in SMART goal format Determine checkpoints Defines the length of time between the start of the work and the projected long-term outcome. Develop an interim outcome for the first checkpoint/Cycle Determine action steps Identifies people responsible, timeline, resources needed, evidence of completion, and data collected
Long-Term Outcomes To set a long-term outcome, review and reflect upon the following: Vision Statement Mission Statement Analysis Results 5
Achievable Long-Term Outcomes How long will your team work together? Time What do you want to accomplish? Task Is your outcome SMART? Terms 6
