Clinical Trial: Sewing Needle Microcapsulotomy vs. Small Needle Aspiration to Avert Argentinian Flag Sign at TOSCON 2024

What is not known ? In reality its not the raised intralenticular pressure the only factor responsible for the extension of tear It is the Configuration of the initial puncture created, to open up the anterior capsule by the sharp tip hypodermic needle or rhexis forceps Puncturing the anterior capsule: with an ultra sharp beveled hypodermic needle, or sharp tip rhexis forceps results into a small linear cut with open edges along the entry
Hypothesis Henceforth we came up with a hypothesis that if we were able to manually create an opening in the anterior capsule which had a round and regular configuration, instead of a linear cut, we may provide resistance against the disruptive forces, thus preventing Argentinian flag sign. Materials and Methods Prospective Randomized Clinical Trial 106 Cases of White Intumescent Cataract Cases, pupil dilatation of 6mm or more and clear cornea Divided Equally into 2 Groups Single Surgeon Group A Small Needle Aspiration Group - B New technique of Sewing Needle Microcapsulotomy Success Surgeon Able to achieve a Capsulorrhexis of 5.25 to 5.50 mm

Conclusion :: In intumescent white cataract, it is primarily linearcut configuration of the initial prick , which has the potential to spontaneously extend along its open edges, leading to Argentinian flag sign. Sewing needle microcapsulotomy with round regular configuration, like a capsulotomy in true sense, provides resistance against the disruptive forces triggered by the high intralenticular pressure. Initial decompression of capsular bag facilitates safe and controlled creation of a capsulorhexis of a desired size with higher level of efficiency and safety THANK YOU
