ADHD STANDARD OPERATIONAL PROCEDURE (SOP) March 2020
What are the aims of todays session? To provide an insight into the ADHD Pre- Diagnostic Pathway. To understand the roles and responsibilities of those involved with following the ADHD Pre- Diagnostic Pathway. Know when and who to contact for support.
THE ADHD PATHWAY The ADHD Pre-Diagnostic Pathway has had several forms over the past few years. The current SOP (Standard Operating Procedure) in place is a detailed description of who does what and when . Each base should have a copy of the SOP available for duty and staff to access.
THE INITIAL REFERRAL. The initial referral is only accepted electronically via email. (Response automatically generated once opened.) DUTY is responsible for triaging the referral to ensure this is appropriate and the correct form is used: check the form for signatures from both parent/school and that all information requested on the referral form has been completed eg parent concerns. Once the referral has been triaged, and accepted, it is added to the 1 Referral in waiting list and the area eg Birkenhead is written in note section. If this is a dual referral or CP/CIN/CLA, or the child has a named paediatrician this will also be added to the notes section and a task sent to the allocated School Nurse to share. Copy the referral form to S1 Records and task team ADMIN to request ADHD Pre- Diagnostic Pathway is commenced.
WHAT HAPPENS NEXT? Within 5 days ADMIN staff will generate the initial letter and send this to the parents with Conner s questionnaire (s) as per SOP and also prepare and send Conner s questionnaire(s) to school as per SOP. The Conner s forms are stamped with a return date of 3 weeks and then moved to the 2 Conner s waiting list. The due date of 3 weeks is entered in the due date column by ADMIN. If Conner s forms are not returned from all parties by the 3 week date ADMIN will generate a letter and send this to both parent and school giving a further date of 2 weeks, sharing that their son/daughter will be discharged from the pathway if the Conner s forms are not returned by this final date. If the Conner s forms are not received by this new date a discharge letter will be sent by dedicated ADHD ADMIN Officer Helen Bristow, following liaison with the School Nurse Assistant, and the ADHD Pre-Diagnostic Pathway will end.
WHAT TO DO WITH THE CONNERS FORMS. Once all the appropriate Conner s questionnaires have been received these are to be scored by ADMIN. When the Comparative report is coped to the S1 records ADMIN will then task the School Nurse Assistant (SNA) to inform her/him. The SNA will review the report. If the Comparative report is UNDER 65 then the SNA will task ADMIN back requesting the Discharge letter on S1 is sent to both parent and school by ADMIN.
IF CONNERS SCORE OVER 65. If the Comparative report is OVER 65 the ADHD Pre-Diagnostic Pathway will continue and the SNA will contact parent to gain consent for a referral to the Parent Skills Training with the ADHD Foundation. Once the email has been sent to the ADHD Foundation (Please add delivery receipt) the SNA then moves the child/young person to the 3 Skills waiting list and the date of the referral added to the comment section and change the due date for 4 weeks.
SKILLS TRAINING CONFIRMATION RECEIVED. Once confirmation of attending the Parent Skills Training is received from the ADHD Foundation Helen Bristow (ADHD Dedicated Admin) will move to either 4a - Classroom Observation (CO) Under 6 waiting list or 4b QB Over 6 waiting list. If for CO Helen Bristow will send a task to the SNA to request this is arranged. If for a QB test Helen Bristow will book an appointment and send an appointment letter to parents/carer, updating the waiting list with this information. Helen Bristow will check paediatric referral is completed within records and if not available will send a task to the SNA to request this is done prior to CO/QB.
REFERRAL TO THE COMMUNITY PAEDAITRICIAN. When the QB test has been completed the report is added to the records by the QB Screener along with a QB Behavioural Report. The child/young person is then moved to the 5 Paediatric Referral waiting list by the QB Screener. If CO completed SNA will move young person to 5 Paediatric referral waiting list. All the relevant information is then sent via the Paediatric secure email by Helen Bristow (ADHD Dedicated Admin) along with the Paediatric Referral form, Comparative Report, Classroom Observation report or QB report, QB behavioural report (if completed), the original 0 19 referral form and confirmation of attendance at the Parent Skills Training attached. This is the final action and once completed the ADHD Pre-diagnostic pathway ends.
CHESHIRE ADHD PATHWAY If a child or young person attending a Wirral school has a Cheshire GP then the SCHOOL will need to make a referral to Cheshire services for ADHD/ASDD/DCD or OTHER. There is a pack available to share with school s (These have already been shared with Birkenhead Private School and Grammar Schools in Wirral.) with all relevant forms in as follows - Cheshire Behaviour Pathway.
