Focus for 2023/24 Establish clear metrics for each programme and ensure they encompass national requirements. Publish our year in review for Access to Care Establish 3 key priorities for each of the programmes (up to 12 in Major Health Conditions) to ensure we can deliver outcomes and improvements in the next 12 months. Deliver on our current priorities including (but not limited to): Implementing the learnings from the paediatric DNA project Opening the first Community Diagnostic Hub in West Yorkshire Evaluating our Wellbeing Hubs; Rethinking Pain services; and community health checks. Supporting the go live of heart failure management, and the pre-hab offer for newly diagnosed cancer patients Develop a tailored diabetes service for young adults and implement the new PCN model of care Evaluate MyCare24 and deliver respiratory step down virtual ward
