Develop and implement a system to place COVID-19 symptomatic or positive individuals who cannot self-isolate into housing. The Housing Triage Team identifies people in need, conducts placement questionnaires, and facilitates housing arrangements, transportation, and testing if required. The team also manages referrals from community partners, health providers, hospitals, and health agencies, ensuring safe isolation for individuals until they are no longer a risk. Criteria for placement include confirmed COVID-19 symptoms, positive test results, or awaiting testing, coupled with the lack of resources for self-isolation in current living situations. The goal is to flatten the COVID-19 curve by isolating affected individuals who lack access to suitable housing.

HOUSING TRIAGE TEAM Goal Flatten the COVID-19 curve Task Develop and implement a system to place people into housing who are COVID-19 symptomatic, positive, or waiting test results that cannot self isolate Prior to process and staffing being identified, no clear means to respond to people who were COVID-19 positive and not able to self-isolate

  3. Goal statement: Flatten the curve by isolating COVID-19 presumptive and COVID-19 positive individuals who don t have access to housing. COVID-19 Response Housing Placement Individual is COVID-19 symptomatic or positive and can t self isolate Community Partner + Health Provider Hospital Discharge Hospital ER CAC/REC WCHD Epi Team Referral and consent form complete and submitted to Housing Placement Team Housing Triage Team conducts housing placement questionnaire. Can this person safely isolate? Housing Triage team makes appropriate housing placement and schedules transportation and testing if needed. No Yes Edison Housing (300 beds) WellCare (43 total beds) Individual self isolates Presumptive - housed until test results received housed in isolation. Positive - housed until no longer a risk (up to 14 days) Negative Discharged Updated 4/24/20

  4. HSA HOUSING PLACEMENT TEAM Triage Team Manager Oversees team and develops key materials Responds to changes in status of people placed in housing Develops procedures, forms, contact lists, etc Triage Team Lead Rotating staff based on availability Received referral calls and coordinated placement, transportation, medication, etc Caseworker/HSSS Case management Operational hours: 8 am to 5 pm, 7 days a week

  5. CRITERIA FOR PLACEMENT Criteria for submitting a referral for requested placement through the Washoe County Human Services Agency Housing Placement Team: Individual is in need of self-isolation due to one of the following: EPI or Hospital or Partner agency confirmed COVID-19 symptom presentation and Has a Positive COVID-19 Test Result or Is awaiting COVID-19 Testing Result AND Individual does not have access or resources to self-isolate in another location due to: Currently residing in location that does not allow for maintaining appropriate self-isolation practices (e.g. shelter, multifamily residence, treatment center, shared bedroom, etc.) Currently Homeless AND Individual must also be able to perform all Activities of Daily Living (ADL) & Instrumental Activities of Daily Living (IADL) and is NOT in need of 24-hour medical care or medical equipment needing on-site staff oversight or supervision.

  6. PLACEMENTS TO DATE As of 4/24 16 referrals received 1 person provided in home case management WellCare Housing 11 placements to date over the course of the contract 6 people currently housed Edison Housing Property manager, food service and security has been procured Additional site supports such as medical and communications mechanisms with property manager around placements are being identified
