Jiao, a 74-year-old Chinese woman, was admitted to the hospital with chest pain and diagnosed with a mild myocardial infarction. She needed surgery due to severe plaque buildup in her coronary arteries. High cholesterol levels led to a restricted diet plan. Language barrier challenges arose when the dietitian couldn't confirm Jiao's understanding of the diet plan, resulting in Jiao refusing to eat the provided meal.

  1. Jiao is a 74-year-old Chinese woman who was recently admitted to the Robert Packer Hospital with chest pain. Her husband reported that after a long day of visiting with their grandchildren she began to act strange and seemed short of breath so he decided to bring her to the emergency room. In the ER Jiao was given a EKG and blood was taken to run laboratory tests. She was also administered oxygen. "Case Study #1" by Dr. Michael Kobre is licensed under CC BY 4.0

  2. After the EKG and blood work was completed, the doctors treating Jiao had determined that she had a mild myocardial infarction. Once Jiao was stabilized, the cardiologist was called and Jiao was admitted to the ICU for further treatment, and observation. The next day, Jiao was evaluated by the cardiologist and it was found that she indeed had a heart attack and four of the coronary arteries in her heart had severe plaque buildup and needed to be cleaned out.

  3. Jiaos surgery was scheduled, and more blood tests were performed showing that Jiao s cholesterol levels were very high. While awaiting her surgery in the ICU, Jiao was visited by the hospitals Dietitian who explained to Jiao that she was being put on a restricted diet to help her lower her cholesterol levels and that she needed to continue following the new diet to help prevent further damage to her heart and cardiovascular system.

Jiao seemed to understand, she nodded her head when spoken to but the dietitian had no way to confirm whether Jiao understood because Jiao did not speak English very well and the dietitian did not speak or understand Chinese at all. Later in the day Jiao s lunch was delivered, and when she opened the cover on her meal she was faced with a cup of decaffeinated tea, some a slice of plain toasted wheat bread with a grape jelly packet, a half a grapefruit, and a bowl of total cereal with skim milk.

  5. Jiao looked at the tray and quickly put the lid back over the food refusing to eat anything.
