  3. Nutrition Requirements for Fluid Milk USDA Memo, SP 29-2011. Persons over two years of age should consume fat- free or low-fat fluid milk. Fat-free and or Low-fat (1%) milk (unflavored or flavored)

  9. CNPweb Monthly Claim Monthly reimbursement Claims are recommended to be submitted through the CNPweb by the 10thday of the month following the claim period. Original claims must be submitted within 60 calendar days following the claim month.

To file a program complaint of discrimination, complete the USDA Program Discrimination Complaint Form, (AD-3027) found online at: http://www.ascr.usda.gov/complaint_filing_cust.html, and at any USDA office, or write a letter addressed to USDA and provide in the letter all of the information requested in the form. To request a copy of the complaint form, call (866) 632-9992.
