Two Plan Options: Managed Medical Care Plan (MMCP) In-network deductible = $350 per individual and $700 per family In-network coinsurance = 90% (you are responsible for 10% of the allowed amount for your services once you reach your deductible) Office visit copays: PCP: $25 Specialist: $40 Urgent Care: $25 ER: $100 Comprehensive Healthcare Benefit (CHCB) In-network deductible = $350 per individual and $700 per family In-network coinsurance = 80% (you are responsible for 20% of the allowed amount for your services once you reach your deductible) No copays Office visits are subject to your deductible and coinsurance

It helps identify problems earlier, when they re easier to treat. It considers your family history and conditions you already have. It even looks at your lifestyle behaviors. Cancer Prevention and Screenings Cancer screenings, from colonoscopies to mammograms are key to cancer detection. Screening tests can help find cancer at an early stage, before symptoms appear. When abnormal tissue or cancer is found early, it may be easier, to treat or cure. By the time symptoms appear, the cancer may have grown and spread and can make the cancer harder to treat or cure.
