  1. Early Help Hubs Swansea Working together to support children, young people and families to live happy, healthy, and safe lives by having access to the right help, at the right time if and when they need it. .

  2. Key Principles: In Swansea Early Help Hubs want to ensure that children and families have access to the right support at the right time from the right person regardless of age and location. Our Hubs and workflows have been developed in line with demand and what matters to families. The Early Help Hubs will be working within the signs of wellbeing framework supporting wellbeing using a locality based hub structure and a single point of contact for professionals. The Early Help Hub model builds on and increases the previous family support provision, formally known as Team Around the Family, Family Partnership Team and Evolve. By co-locating and bringing these services into one structure we can ensure that support is available regardless of age or location, that it is integrated with partners and utilises community assets to provide: o Child or early years work o Youth work o Whole family work Ultimately the Early Help Hubs will include multi agency representation providing a single, integrated resource for family support.

  5. Who will the Early Help Hubs work with? Children young people and their families where there are identified wellbeing needs that the family need assistance to achieve. It is really important that we think about what outcomes the child and family want to achieve and what outcomes you identify How can family s access support directly from the Early Help Hubs? Children, young people and their families can access support from the EHH by either ringing them on 01792 635400, e-mailing them at earlyhelphubs@swansea.gov.uk or by attending at one of the community drop ins. For telephone and e-mail enquiries they will be directed to the EHH that covers their home address and talk to one of the staff about what matters to them. This will be the start of a proportionate assessment to identify the right service at the right time to help families live happy, healthy and safe lives. What will the Early Help Hubs do? As part of the proportionate assessment the EHH will provide information about our services and others that may be able to help families. We provide advice in relation to how we or other services can support them and assistance if the services we provide are the right ones to help the family achieve their wellbeing outcomes.

  6. What will assistance from the Early Help Hubs look like? Assistance could include a lead worker to help the family identify their own wellbeing goals and provide support in order to achieve them. We will do this by speaking to the family to understand what matters to them in order to assist them in developing their own wellbeing goals/personal outcomes. We will encourage them to consider what strengths they have within their family, friends and community and develop a plan in order to achieve the wellbeing goals. Support to achieve these goals may be provided directly by the EHH or we may pull in the expertise of other universal or third sector organisations if they are considered the right service to meet the identified need. Assistance could also include linking the family in with one of our emotional health and wellbeing workers, a group work offer or a 121 community drop in, dependant on the identified wellbeing needs. If through our intervention we identify safeguarding concerns, which indicate that a child is likely to suffer significant harm, the family would be assisted to access the right support from statutory child and family services through the step up process. If we identify that a child or young person has suffered significant harm then the usual process of contacting the Single Point of Contact on 01792 635700 will be followed without delay.

  7. What type of groups are available for children young people and their families to access? Family Lives Group Support to help parents better understand the needs of their children. Suitable for parents of children aged between STEPS Group A programme that focuses on building adults self-esteem, confidence and efficacy whilst understanding how their life experiences have shaped them PASP Group A programme for parents who are experience conflict in their relationships in terms of co-parenting their children ALN Group Information, advice, guidance and support for parents with children who have emerging needs/diagnosis. Emotional Health and Wellbeing To support the development and implementation of plans and strategies to support children and young people s emotional/mental health and wellbeing via 1:1 and group work. ASD Parenting Support A programme that takes families on a journey of understanding Autism/living with Autism. Aims to give families the skills for lifelong learning so that they may be able to grow and adapt to the ever changing environment. Young Person Residential To support young people in gaining perspective, recognise and respond to the unique challenges experienced during this stage of life. Family Residential Family Residential are used to support families in gaining perspective, and help them exam the social foundations of their family unit. The residential aims to bring families together in a new and different environment; temporarily removing external family pressures allowing staff and families to work together.

  8. What support can families access if they have an allocated Social Worker? The EHH can also support children and families where there are eligible care and support needs that can only be met by the provision of statutory services. This support can be provided via the delivery of group work or 121 sessions through our community drop in. The support from the EHH can be identified as the right service for the family and be part of the child s plan with clear outcomes and timescales. What support is available for families from the community drop in? If families are looking for support around understanding their children s behaviour or strategies to manage behaviour then the community drop in could be the right offer for them. Where parents needs some information, advice or guidance around supporting their children s age and stage of development, emotional and physical development or support with better understanding their needs, then the community drop in will offer between 4-6 weeks of support in a venue within their locality. What will the community drop in provide for the organisations that host them? Currently we have the ability to host community drop ins at our EHH s in Gorseinon, Blaenymaes, Townhill and Stadwen in Llansamlet. We can also utilise space in the family centres at Mountain View in Mayhill and the Penlan Integrated Children, as well as most Flying Start Settings. We have also sent out an expression of interest to Primary Schools in Swansea. We are particularly interested in those that sit in the areas where we do not already have an established location for a drop in. Any organisation with the space to host a drop in will benefit from a named link worker, frequent and consistent support from the EHH covering their area, localised and responsive support for parents/children accessing your venue, access to skilled and knowledgeable staff and access to multi agency training
