Sexual Health Promotion Work in the community to improve the sexual health of vulnerable young people and adults and populations at increased risk of poor sexual health Young people with identified vulnerabilities, unintended pregnancy or sexually transmitted infections MSM of all ages at risk of poor sexual health outcomes Individuals from BME populations at increased risk of HIV transmission 7
Sexual Health promotion Aim increase an individual s self-awareness and provide brief advice or brief interventions to those identified at high risk. Initial health assessment to identify needs and level of sexual risk 1-1 brief intervention and behaviour change programme to increase the use of effective contraception methods and to promote safer sex practices onward referral to specialist services as needed Advocacy support to access specialist Sexual Health Services (Health Advisers, SH Nurses, HIV / GUM / SRH doctors) point of care tests (HIV and Syphilis), self taken samples for Chlamydia and Gonorrhoea. Condom distribution and pregnancy testing Onward referral to community support organisations e.g. Youth Support Services and Chrysalis (Supporting transitioning transgender people) 8
Outreach Nurse Outreach nurses visit clients who are unable to attend clinic alone or with a parent/guardian/carer Aim: to support their eventual attendance at a clinic. Suitable clients: clients aged under 18 Clients with a recognised vulnerability and are at risk of unplanned pregnancy or sexually transmitted infections. 9
Sexual Assault Treetops: Sexual Assault Referral Centre (SARC) for all people living in Hampshire and Isle of Wight. Services: Crisis Worker team: confidential telephone advice, independent advocacy and support with practical matters, introductions to other support services, e.g. Rape Crisis/ Victim Support, Forensic Medical Examination: to collect evidence, address medical needs inc EC / PEPSE. HCP s requiring advice inc medical advice: speak to a Crisis Worker 24/7, 365 days a year on 0300 123 6616. If non-urgent advice; call Mon Sat 9am and 4pm 10
HIV referrals Urgent HIV referrals new diagnosis health concern with an existing HIV patient Look at website for details of how to refer urgently Non Urgent HIV referrals HIV patient transfer of care non urgent query about an existing patient Usual referral form emailed to usual address 11
Psychosexual Referral GP referral only brief psychological-based sex therapy interventions (max 6 sessions after initial assessment) for sexual problems in individuals and couples Problems: Loss of desire/lack or loss of libido Orgasm problems Erection problems Premature/delayed ejaculation Dyspareunia/Vulvodynia Vaginismus/non-consummation Physical cause should be ruled out (detail these investigations in referral) 12
Psychosexual Counselling Criteria over 16 years old not currently engaged in counselling with another provider, ready and able to commit to a course of therapy lasting several months, psychologically stable (any underlying mental health issues appropriately treated) not dependant on drugs or alcohol 13
What can sexual health not do: Cervical smears Working with commissioners currently Contraception for non contraceptive reasons IUS for menorrhagia / HRT If IUS has been fitted at age 45 or over we will not replace at 5 years even if needs exchange for HRT. CHC / injection / implant in non sexually active females for period control e.g. learning difficulties Prescribe HRT We can prescribe and issue repeat CHC/POP but we would prefer not to More appointments to see the more complex contraception and STIs 15
What psychosexual counselling cannot help with: Erectile dysfunction General relationship issues Sexual addiction/sexual offending Historical sexual abuse/sexual assault Gender Identity 16
