2. What is PACOP? Clinical Bites of Health & Aged Care National program to improve palliative care in Residential Aged Care PACOP = Palliative Aged Care Outcomes Program Funded by Australian Department Companion program to PCOC

PACOP = Companion Program to PCOC Problem Severity Score (PSS) Symptom Assessment Scale (SAS) Clinical Bites Resource Utilisation Groups-Activities of Daily Living (RUG-ADL) Australia-modified Karnofsky Performance Status (AKPS) Rockwood Clinical Frailty Scale (RCFS) Palliative Care Phase (PHASE)

4. Data Informs Development of PACOP Approximately 60,000 people die each year in RAC 38% die within 1 year of admission Clinical Bites 19% die within 3 months of admission PACOP Commences screening for palliative need from admission Continue to screen 3 monthly (at a minimum) Once palliative, increase assessment frequency & respond using evidence-based protocol

8. Why should RAC participate in PACOP? Evidence based Clinical Bites Support & empowers staff Established framework No resident falls through the cracks Facilitates CQI / ACQS Common language
